- ✪ ✪ ✪ ✪ ✪Can the Drs kindly relinquish the documentation of the shots my son had here. Tammy will not do it. My son cannot receive shots without proof he had shots here. He is only 8 months. My husband has called several times. This is very imperative he gets his shots please. He is just a baby. I am asking someone to forward his record we already signed medical release.
